What happened to the header heat shield?

Yep....that's a PZEV manifold. To regulate their internal temperature better, they are double-walled and do not need an insulator.
The mounting plates are the same for the PZEV and the U.L. (standard) manifolds.....hence the reason for the mounting tabs you see. However, if it were a U.L. manifold, it would have backing-nuts tack welded to those tabs to accommodate bolts for the insulator. Having a common mounting plate simplifies things for the supplier who produces the manifolds.
BTW, that insulator on the side is there to help keep some of the exhaust heat away from the A/C compressor.
As I said earlier, a PZEV manifold looks rather unfinshed. So thinking something was missing would be an easy mistake.
